Housing Case Manager (Rapid Re-Housing)

Position Summary 
The Housing Case Manager position exists to provide case management services to participants in the Rapid Re-Housing (RRH) programs. The goal of the program is for participants to be able to pay full rent in a market rate unit after being in the program by receiving case management, education and employment assistance, health and wellness support, and a focus on increasing income. Ensures participants meet the eligibility criteria of the Rapid Re-Housing program, i.e., homelessness status, income verification, etc., and any additional needs are met while receiving YWCA assistance. These duties are performed both in a general office environment and in the field and community where program participants reside.

Position Duties & Functions
                     
o    Facilitates direct services and housing assistance for the Rapid Re-Housing program (e.g. intake, counseling, advocacy, deposit and rental assistance, referrals, and case management, etc.) 
o    Facilitates screenings, assessments, safety and financial planning and referrals for all participants in the program
o    Assist with participant identifying and locating housing and services that will help them implement their goals.
o    Work with participants to enroll in public benefits to which they are entitled
o    Teach practical financial skills, including developing a household budget, paying bills in a timely manner, and opening and maintaining a bank account
o    Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. tenant, landlord, referral source, collaborating agencies, debtors and creditors). 
o    Assist participants in development of a strength-based/solution-focused individualized goal and action plan that promotes permanent housing and self-sufficiency; develop an effective, timely referral network in order to ensure ongoing direction and support as needed.
o    Identify participant strengths and barriers to stability and assist participants to reducing barriers and linking to resources and services.
o    Monitors data security and ensures consistent implementation daily
o    Facilitates existing partnerships, collaborations, and relationships with other agencies or providers to ensure the best delivery of services to participants in our Rapid Re-Housing program (e.g. Landlords, CILO for disabilities, Lewis Center for housing, etc.)
o    Builds new partnerships, collaborations, and relationships with other agencies or providers to acquire the best services for participants in the Rapid Re-Housing.
o    Facilitates at least 2 community education or professional trainings in the community annually to promote our Rapid Re-Housing Program. 
o    Maintains accurate, complete, and up-to date participant files and case notes; Osnium and Homeless Management Information System (HMIS) entry and all other requirements to ensure accurate statistical data. 
o    Prepare reports and other paperwork per established program standards. 
 

Skills & Abilities
•    Must be knowledgeable of Microsoft Office (Word, Excel, PowerPoint, Outlook)
•    Must have excellent verbal and written communication skills
•    Must be prompt; timely and responsive to needs of fast-paced organization
•    Must be able to handle confidential information discretely
•    Must be able learn quickly and have strong problem solving and critical thinking skills.
•    Must be able to work independently and with diverse populations and able to meet clients in their homes
•    Requires knowledge and belief in “Housing First” philosophy and strategies.
•    Must maintain a positive work environment
•    DCF Domestic Violence Program Core Competency and privilege status obtained within first 90 days.

Education

•    Bachelor’s degree in Social Work, Criminal Justice, Sociology, Psychology, or related Human Service field

Experience
•    Minimum 1 -year experience of case management, preferably with individuals or families impacted by homelessness, those experiencing domestic violence, or participants working impacted by trauma, providing advocacy and/or housing services and familiarity with community resources, networking and collaboration
•    Minimum 1 -year experience utilizing client database systems (i.e. Osinum, Homeless Management Information System (HMIS) or similar data systems) for data entry and reporting preferred
•    Familiarity with HUD Continuum of Care (CoC) Program regulations.
•    Must have a valid driver’s license

           
SPECIAL REQUIREMENTS AND PHYSICAL DEMANDS:
•    Ability to exert physical effort which may involve some lifting, carrying, pushing and/or pulling of objects and materials of light weight (up to 20 pounds), bending, stooping, stretching, squatting, and sitting.
•    Travel required in the Palm Beach County area.


Starting Salary Range: $36,000-$46,000
